ABSTRACT

Objective:To construct endocrinology nursing subspecialty model and explore its clinical effect.Methods:In December 2018, the organization structure of endocrinology nursing subspecialty was constructed in the Drum Tower Hospital Affiliated of Nanjing University Medical School and applied in clinic. In this model, the data of 2018 were taken as the data before application and the data of 2020 were taken as the data after application. The comprehensive ability of nurses, nurse satisfaction, related nursing workload and scientific research ability of nurses were compared before and after the application of the model.Results:After the application of subspecialty nursing mode, nurses′ comprehensive ability score was (92.00 ± 2.36) points. Compared with (84.25 ± 3.24) points before implementation, the difference was statistically significant ( t=-9.46, P<0.01); nurses′ satisfaction evaluations including specialty development (7.92 ± 1.41), self-quality improvement (8.00 ± 1.69), work pressure (6.42 ± 2.67), salary and welfare (3.96 ± 0.85), compared with (5.79 ± 2.31), (6.17 ± 2.82), (8.33 ± 1.50), (2.88 ± 1.59) before implementation, the difference was statistically significant ( t values were -3.86--2.73, all P<0.05). The annual workload of related nursing increased and the scientific research ability of nurses was improved. Conclusions:The application of endocrinology nursing subspecialty mode is beneficial to improve nurses′ comprehensive ability of clinical work, improve the level of specialized nursing, improve the quality of nursing service and promote the improvement of economic benefits, which is worthy of clinical promotion.

ABSTRACT

Circular RNA is a class of non-coding RNAs, which are covalently closed and circular at both ends, showing dissimilar characteristics from linear RNA. Several studies have shown that circular RNAs play an important role in the occurrence and development of primary hepatic cancer. By combining with the latest research progress of this field at home and abroad, we summarized the mechanism regulating the occurrence and development of liver cancer, abnormal expression, and as potential molecular markers for disease diagnosis and treatment.

ABSTRACT

Objective@#To explore the effect of acupoint application combined with conventional anti-tuberculosis treatment on pulmonary tuberculosis and its effect on the immune function of patients.@*Methods@#A total of 64 patients with pulmonary tuberculosis who visited Hangzhou Red Cross Hospital from March 2016 to September 2017 were selected in the study.The patients were randomly divided into control group (n=30) and observation group (n=34) according to the digital table.The clinical curative effect (obvious absorption, absorption, no change and deterioration), lung function (FVC, FEV1 and FVC/FEV1), immune function(CD3+, CD4+, CD8+, CD4+/CD8+, IgC, IgA, IgM) were compared between the two groups.@*Results@#The absorptive rate of the control group was 46.67%, which was significantly lower than that of the observation group(76.47%, χ2=6.04, P<0.05). After treatment, the levels of FVC, FEV1 and FVC/FEV1 of the two groups were significantly increased compared with before treatment..After treatment, the levels of FVC, FEV1 and FVC/FEV1 in the observation group were (2.20±0.48)L, (2.50±0.34)L, (87.44±13.60)%, respectively, which were significantly higher than those in the control group [(1.63±0.32)L, (2.02±0.44)L, (80.28±12.66)%] (t=6.607, 5.687, 2.558, all P<0.05). After treatment, the CD3+, CD4+, CD8+, CD4+/CD8+, IgC, IgA, IgM levles in the observation group were significantly increased compared with before treatment (all P<0.05). After treatment, the CD3+, CD4+, CD8+, CD4+/CD8+, IgC, IgA, IgM levles in the observation group were (73.25±6.17)%, (38.65±5.75)%, (36.58±3.17)%, (1.52±0.65), (15.49±1.49)g/L, (3.07±1.30)g/L, (1.94±0.50)g/L, respectively, which were significantly higher than those in the control group[(68.43±6.4)%, (34.72±5.68)%, (35.02±3.08)%, (1.16±0.78), (12.61±1.64)g/L, (2.23±0.90)g/L, (1.68±0.35)g/L] (t=3.590, 3.223, 2.340, 2.340, 8.594, 3.550, 2.846, all P<0.05).@*Conclusion@#Conventional anti-tuberculosis chemotherapy drugs combined with acupuncture has better effect than conventional anti-tuberculosis chemotherapy drugs.It can significantly improve the immune function of patients, improve the success rate of treatment, and speed up the improvement of the patients' condition.

After treatment, the CD+3 , CD+4 , CD+8 , CD+4 /CD+8 , IgC, IgA, IgM levles in the observation group were (73.25 ± 6.17)% ,(38.65 ± 5.75)% ,(36.58 ± 3.17)% ,(1.52 ± 0.65), (15.49 ± 1.49)g/L,(3.07 ± 1.30)g/L,(1.94 ± 0.50)g/L,respectively,which were significantly higher than those in the control group [( 68.43 ± 6.4 )% , ( 34.72 ± 5.68 )% , ( 35.02 ± 3.08 )% , ( 1.16 ± 0.78 ), ( 12.61 ± 1.64)g/L,(2.23 ± 0.90)g/L,(1.68 ± 0.35)g/L] (t=3.590,3.223,2.340,2.340,8.594,3.550,2.846,all P<0.05 ). Conclusion Conventional anti - tuberculosis chemotherapy drugs combined with acupuncture has better effect than conventional anti-tuberculosis chemotherapy drugs.It can significantly improve the immune function of patients,improve the success rate of treatment,and speed up the improvement of the patients'condition.

ABSTRACT

Objective To investigate the efficacy of ultrasound-guided transveses abdominis plane block ( TAP) on anesthesia and postoperative analgesia in patients undergoing gynecological laparoscopic surgery. Methods From January 2015 to July 2017,60 patients aged 46 -68 yeas,of American Society of Anesthesiologists (ASA) physical status I-II,scheduled for elective gynecological laparoscopic surgery in Nanxiang Hospital of Jiading District of Shanghai were randomly divided into control group( A group) and ultrasound-guided TAP group( B group) ,with 30 cases in each group,In B group,ultrasound-guided TAP was performed 30 min before induction of anesthesia. The patients in both two groups received patient-controlled intravenous analgesia ( PCIA) for 48 h started from 10 min before the end of surgery. The VAS score ≥4points,sufentanil 5μg was injected intravenously. The consumption of anesthesia drugs during operation,time of consciousness and extubation,the consumption of sufentanil per hour in 24h after operation,requirement for rescue medication,and the adverse reactions within 24h after surgery were also recor-ded. Results Compared with those in A group,the consumption of propofol[(210 ± 32) mg vs. (324 ± 45) mg], sufentanil[(31 ± 3. 6)μg vs. (42 ± 3. 8)μg] in operation and consumption of sufentanil per hour in 24h after operation [(3.2±0.5)μgvs.(5.4±0.7)μg?inBgroupweresignificantlylower(P<0.001).Thetimeofconsciousness [(12 ± 3)min vs. (20 ± 5)min]and extubation[(15 ± 4)min vs. (25 ± 7)min]were significantly reduced in B group (P<0. 001). The requirement for rescue medication(13% vs. 40%) and the incidence rate of nausea(10% vs. 33%) and vomiting(0% vs. 10%) within 24h after surgery were significantly decreased in B group compared with those in A group (P<0. 05). Conclusion Ultrasound-guided TAP performed in patients undergoing gynecologicallaparoscopic surgery can reduce the consumption of anesthesia drugs in operation and optimize postoperative analgesia.

ABSTRACT

Objective To compare the clinical effects of oral proton pump inhibitor and intravenous proton pump inhibitor in preventing re-bleeding for patients with peptic ulcer.Methods From January 2014 to December 2015,149 patients with peptic ulcer bleeding who came to our hospital for treatment and were successfully stanched by endoscopic therapy were chosen as study subjects.They were randomly divided into oral group and intravenous group according to random number table.The clinical effects,including re-bleeding rate,mortality and volume of blood transfusion were compared between the two groups.Results The re-bleeding rates within 20 days of the oral group and intravenous group were 5.6% and 5.1%,respectively,and there was no statistically significant difference between the two groups (x2 =0.02,P > 0.05).The hospital stay of the oral group and intravenous group were (4.0 ± 0.4) d and (2.1 ± 0.6) d,respectively,and there was statistically significant difference between the two groups (t =22.90,P <0.05).The volume of blood transfusion of the oral group and intravenous group were (1 001 ±284.2)mL and (601.0 ± 197.9) mL,respectively,and there was no significant difference between the two groups (t =0.90,P > 0.05).Both two groups had no death within 30d and during surgery.Conclusion For the patients who were successfully cured by endoscopic therapy,the clinical effect of oral proton pump inhibitor is similar to the effect of intravenous proton pump inhibitor,and oral proton pump inhibitor is more easy operating and lower economic burden than intravenous proton pump inhibitor.

ABSTRACT

Objective Aspirin is a non steroidal anti-inflammatory drugs, whether or not which does harm to renal function in the course of using, there is a big debate at home and abroad.In this paper, The aim of this study was to observe the effects of different doses of aspirin on renal function in elderly patients with cardiovascular disease at different ages.MethodsWe Selected a total of 288 patients with cardiovascular disease from January 2014 to March 2016 in Zhejiang Rongjun hospital.There were 288 cases of cardiovascular disease, including 144 cases of normal uric acid, 144 cases with hyperuricemia (including gout).The two groups according to the age of 60 to 70 years, 71 to 80 years old, over 80 years old respectively into three subgroups, each subgroup of 48 cases;Each subgroup was randomly selected without the use of aspirin (0mg/d) and the use of different doses of aspirin (50mg/d, 100mg/d, 150mg/d) for a total of four groups, each which is 12 patients.We detect patients routine blood tests, liver and kidney function, blood urea nitrogen (BUN), blood creatinine (SCR) levels and glomerular filtration rate (GFR) (MDRD) and blood and urine beta 2-microglobulin used aspirin before and used after 6 months and 12 months.ResultsBefore treatment, there were no significant differences in body mass index (BMI), blood pressure, hemoglobin, serum albumin and platelet count between various groups of the same age group.After treatment for 6 and 12 months,Uric acid normal patients and high uric acid patients with 0mg/d, 50mg/d, 100mg/d or 150mg/d dose of aspirin has no statistical difference in BUN, Scr and eGFR, 2-microglobulin Ccr, blood, urine beta 2-microglobulin.ConclusionRegardless of uric acid, different doses of aspirin had no significant effect on renal function in elderly patients with cardiovascular disease, and can be safely used.

ABSTRACT

Objective To observe clinical efficacy of probiotics agents in the prevention and treatment of severe pulmonary infection in elderly patients with antibiotic-associated diarrhea (AAD).Methods 60 cases of elderly patients with severe pulmonary infection (more than or equal to 60 years old) were randomly divided into the treatment group ( 31 cases ) and control group ( 29 cases ) , the control group received broad-spectrum antibiotics or using two linked above anti-infection treatment, the treatment group were added with probiotic agent ( lactobacillus complex capsules).The diarrhea, the use of antibiotics and the stool routine, bacteria before and after 5, 10, 15 days of group were observed.Results After 15 days treatment, the number of Escherichia coli in treatment group was lower than that in control group (P<0.05), the number of Bacterium lacticum and Bifidobacterium bifidum in treatment group were higher than those in control group ( P<0.05 ) .The AAD rate in treatment goup was 12.90%, which was lower than 41.38% in control group (P<0.05).There were significant differences in beginning time and duration of diarrhea between two groups (P<0.05). Conclusion The intestinal probiotics reduced induced by antibiotics in elderly patients with severe pulmonary infection , the probiotics agents could redress intestinal flora imbalance, keep the steady state of intestinal flora, and prevent and cure the antibiotic-associated diarrhea.

ABSTRACT

Objective To examine the effects of Shenqi Bufei Tang decoction on the expression of histone deacetylase-2 ( HDAC2) and nuclear factor-κB p65 ( NF-κB p65) in the airway smooth muscle tissues of COPD rats with lung-qi deficiency syndrome. Methods A total of 40 male rats were randomly divided into normal control group,model control group,Shenqi Bufei Tang decoction group,and aminophylline group.The COPD rat model with lung-qi deficiency syndrome was established by intra-tracheal instillation of lipopolysaccharide (LPS) and passive smoking for 28 days.Pathological changes of lung tissues were ob-served under the light microscope and the thickness of the small airway wall and airway smooth muscle ( ASM) layer analyzed by the image analysis.Immunohistochemistry,real-time PCR and Western blotting were used to detect the expressions of NF-κB p65 and HDAC2 in ASM. Results The thickness of the airway wall and ASM,and the expression levels of NF-κB p65 mRNA and protein were significantly increased in the model control group when compared with those in the normal control group ( P0.05). Conclu-sion Shenqi Bufei Tang decoction can inhibit the proliferation of ASM in COPD rats with lung-qi deficiency syndrome,which may be associated with the increased expression of HDAC2 and decreased expression of NF-κB p65.

ABSTRACT

Objective To investigate the influence of HAp-CS composite microsphere scaffold on the in vitro cell behaviors of mesenchymal cells and evaluate its potential application for bone tissue engineering.Methods Nano-hydroxyap-atite (HAp)and chitosan (CS)composites solution were assembled into microsphere scaffold through microfluidic and observed by inverted microscope.Rat bone marrow mesenchymal stem cells were co-cultured in vitro with the microspheres for calculating the adhesion rate for the first 6h.Proliferation rate was measured by cell counting in the next 1,3,6,9 d,respectively,and GraphPad Prism 6 software was used for statistical analysis.The morphology of BMSCs on the surface of HAp-CS composite microsphere was observed by scanning electron micros-copy (SEM)and confocal scanning microscopy.The cells and HAp-CS microspheres were filled into a disc mold and co-cultured for 14 ~21 d to observe the morphology.Results HAp-CS microspheres were observed to be round and with uniform size by microscope.The adhesion rate of BMSCs reached 80% after cultured for 6 h,and proliferation rate reached the highest value when cultured for 6 d.SEM observations showed that BMSCs adhered compactly to the surface of the microspheres,and the microspheres could be connected together through BMSCs.Af-ter co-culturing BMSCs with microspheres for 14 ~21 d,a complete tissue constructs could be formed.Conclusion HAp-CS microspheres are proved to be good scaffolds for promoting BMSCs adhesion and proliferation.Large a-mount of extracellular matrix can be formed to connect microspheres after co-cultured for a certain time,which paves the way for HAp-CS microspheres to be applied for bone regeneration in animal experiments.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the role of sociopsychological factors in occupational hand injury and to provide a reference for intervention measures.</p><p><b>METHODS</b>The sociopsychological factors in 150 patients with occupational hand injury who were admitted to the Department of Hand Surgery from January to June in 2014 and 150 pair-matched normal controls were surveyed using self-made questionnaire and Life Event Scale (LES). A comparative study was carried out using t test, χ² test, and conditional logistic regression analysis.</p><p><b>RESULTS</b>The demographic characteristics, such as length of service, education level, and marital status, showed significant differences between the two groups (P<0.05) ; the LES total score and values of positive life events and negative life events also showed significant differences between the study group and control group (P<0.01) ; single-factor and multi-factor conditional logistic regression analyses showed that length of service, marital status, and negative events score were statistically significant variables (P<0.01).</p><p><b>CONCLUSION</b>Sociopsychological factors play an important role in the occurrence of occupational hand injury.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the survival rate and complications of endodontically treated residual crowns and roots restored directly or by glass fiber post and core or cast metal post and core.</p><p><b>METHODS</b>In this study, 96 patients with a total of 142 teeth were selected. All the teeth were undergone root canal therapy. And then, according to different situations, some of them were restored with glass fiber post and core, another with cast metal post and core, the rest were restored directly in the strict controlled indications.</p><p><b>RESULTS</b>The teeth (135) in 89 patients were divided into three groups. The first group contained 73 (54.1%) teeth restored with glass fiber post and composite resin core. The survival rate reached 97.3% in the average observation period of (5.7 +/- 1.9) years. The second group contained 40 teeth, restored with pure titanium casting post and core. The survival rate reached 87.5% in the average observation period of (6.2 +/- 2.2) years. The last group contained 22 teeth received no treatment. The survival rate reached 90.9% in the average observation period of (5.4 +/- 1.8) years. The success rate had no significant difference among the three groups. The most common complications after restored included root fracture (6.7%), retention loss (2.2%), secondary caries (1.5%) and periapical periodontitis (0.7%).</p><p><b>CONCLUSION</b>Under the premise of high quality root canal treatment and standard restoration treatment, the abutment teeth, whether are restored by glass fiber post and core, cast metal post and core or without post, have a higher survival rate over 5 years of observation period and complications especially root fracture still should attract the necessary attention.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the microscopic characteristic of the bonding interface between resin cement and dentin before and after using Nd: YAG laser, and evaluate the effects of the Nd: YAG laser on the shear bond strength of 3 resin cements to dentin.</p><p><b>METHODS</b>30 human premolars were cut into nearly equal halves, prepared to expose the dentin and randomly divided into two groups: Laser group and control group. The teeth in the laser group were processed with pulse Nd: YAG laser (0.8 W, 10 Hz) for 25 s, and then the teeth were bonded using 3 resin cements (RelyX ARC, Panavia F and RelyX Unicem) to dentin. All teeth in the control group were directly filled with 3 cements. The shear bond strength of specimens was measured. Each fractured specimen was examined under dental microscope. And 6 new human premolars were prepared into standard bonding specimens. After 3 teeth processed with pulse Nd: YAG laser, all teeth were bonded using 3 resin cements. To observe the microscopic characteristics of the bonding interface between resin cements and dentin.</p><p><b>RESULTS</b>Nd: YAG laser on dentin surface could increase bond strength of Panavia F and RelyX Unicem (P < 0.05). And the laser could decrease bond strength of RelyX ARC (P < 0.05). Most of the fractured adhesive dentin surfaces were adhesive failure modes. Scanning electron microscope (SEM) observation demonstrated that Nd: YAG laser irradiation on dentin surface could decrease the hybrid layer and resin tags between the resin cements (RelyX ARC and Panavia F) and dentin. The interface between RelyX Unicem and dentin showed no obvious change, and resin tags were not observed.</p><p><b>CONCLUSION</b>Nd: YAG laser on dentin surface can increase bond strength of Panavia F and RelyX Unicem. Therefore, they can be used together in clinic.</p>

ABSTRACT

Objective To assess the reliability and validity of the Chinese version of the neuropsychiatric inventory (CNPI). Methods The CNPI was administered to 219 caregivers of patients with Alzheimer's disease (AD). Each caregiver was retested 4 weeks after initial testing. Results The Cronbach a coefficient of the total symptom scale was 0.69. The Cronbach α coefficient of the total caregiver distress subscale was 0.72. The Cronbach α coefficient of the entire inventory was 0. 82. The test-retest coefficients ranged from 0.66 to 0.98 (P < 0.01). Principal axis factoring analysis of the symptom subscale yielded a five-factor solution which contributed to 67.0% of the cumulative variance. Factor 1, which included aberrant motor behavior, hallucinations, delusion and irritability had the most significant contribution to the cumulative variance. Principal axis factoring analysis of the caregiver distress subscale also yielded a five-factor solution which contributed to 70.2% of the cumulative variance. Factor 1, which included depression, delusion, sleep/night behavior, aberrant motor behavior, and irritability had the most significant contribution to the cumulative variance. Conclusion This Chinese version of NPI is a reliable and valid tool for measuring neuropsychiatric disturbances in patients with AD.
